To have your medical record transferred to FMC, you will need to complete and sign a medical record release form. Once this form is complete you can either drop it off at the FMC center of your choice or you can mail it to the practice you are requesting it from. You can now access the medical records release form online. Please click here for FMC's Medical Record Release Form. Download, print and complete this form to authorize FMC's receipt of your past medical record. Once received, this information will be reviewed by your FMC physician and will be kept in your FMC chart.
