At FMC we try to make your first appointment as form free as possible. To expedite your visit, please bring your photo identification, medical insurance card and prescription insurance card. You will be asked to complete a patient data form which gives us your demographic information as well as your medical history. You will also be asked to sign a Patient Consent Form that allows FMC to treat you. You can now complete the Patient Data Form before you visit FMC. Click here for the Patient Data Form. Simply print this form, complete it and bring it with you to your FMC appointment.
