Let the Patient Services Representative know you would like to make an appointment. If there is a physician you would prefer to see, please let the Patient Services Representative know and they will advise you of the next available appointment. If you are a new patient or have changed your insurance since your last visit please bring your medical insurance card, prescription insurance card and a picture ID and arrive 25 minutes early to fill out the necessary paperwork. If you are an established patient, please arrive 10 minutes early to update any necessary paperwork. To save additional time you can now download and print our Patient Data Sheet. Bring this completed form with you to your appointment and give it to the Patient Service Representative at check In. You can now also download and print our Medical Records Release Form and our Notice of Privacy Practices.
